Page 1 displays view-only data, taken from across all of the selected patient's Encounters.
When an Encounter is updated, the Face Sheet for that patient automatically displays the updated data.
Displayed data
All data is view-only.
- Data displays in descending order by date of entry.
- Use the scroll bar on the right of each section to view additional entries for that section.
Chief Complaint
From the Chief Complaint Element.- Date = Encounter date.
- Complaint = Patient Complaint Details data are prefaced by (Pt). HPI data are prefaced by (HPI).
Diagnoses
From the Final Diagnoses Element.- Date = Date of Diagnosis.
- Diagnosis = ICD-9-CM Code, Eye (in parentheses), Description.
CPT Coding/Procedures
From the Final Procedures Element.- Date = Date of CPT Code.
- Procedures = CPT Code, Modifier (in parentheses), Description.
Past Surgeries
From the Procedure field of the Surgeries, Laser Surgery and Injection Site Elements, as well as the Final Procedures Element, Procedure Code and description fields of those Procedures that are surgery-related.- Date = Date of surgery.
- Surgery = Surgery type, Eye (in parentheses), Name of surgeon.
C/D Ratios
From the Posterior Segment Element.- Date = Date recorded.
- OD Hz | OD Vt | OS Hz | OS Vt = Measured values for each.
Patient Medications
From the Medication History Element.- Date = Prescription date.
- Drug = Name, Dosage, Sig.
Visual Acuities
From the Visual Acuities Element.- Date = Date recorded.
- Type = Unaided, Spectacles or Contacts.
- OD Dt | OD Nr | OD PH | OD Dt | OS Nr | OS PH | OU Dt | OU Nr = Measured values for each.
